Enrollment Policies

Parents/guardians enrolling their child/children must meet with a Playworks Operation Manager to ensure space availability. Call 952-496-6814. This orientation meeting should take place at least one week prior to your start date. Please let us know if your child has any special needs (i.e. food allergies, asthma, medical alerts, etc.)

What you can expect during the enrollment meeting:
  • Introduction to our teachers
  • A review of the classroom schedule
  • A review of program policies and procedures
  • Child care benefit set up – if applicable
  • Picture taken for parent badge
Documents required for enrollment:
  • Admission Agreement
  • Enrollment/Authorization/Permission Form
  • Immunization Form, which must be signed by a physician if under 15 months old. MUST have before child care start date
  • Individual Assessment of Special Needs
  • Automatic Payment Form
  • Transportation Form – if applicable
  • Child Information Sheet
  • Medical Emergency/First Aid Card
  • Health Care Summary
